Counter Notification – Restoration of Material

  1. Your physical or electronic signature.
  2. Description of the material taken down and its original location.
  3. A statement under penalty of perjury that you believe the material was removed by mistake or misidentification.
  4. Your name, address, and telephone number, and a statement consenting to the jurisdiction of the federal district court for your address, and that you will accept service of process from the person who provided the original infringement notification.

Repeat Infringer Policy
